Basic Steps Overview

·       Login

·       Retrieving Record

Login

·       Start Browser

·       Go to:  https://ter2.dshs.state.tx.us/edeath/

·       ‘Click’ on the User Name Field

·       Enter User Name then press ‘TAB’

·       Enter Password then press ‘ENTER’

 

Using ‘Search’ to retrieve a Record

·        Click on Icon to enter Medical Data Entry Function

 

·          Click on ‘Search’ icon to search for the record

 

·       When the ‘Search Record Screen’ appears, enter the EDR number of the record you want to retrieve

 

Note: TER will send an email notification to the medical certifier which will include the Electronic Registration Number (EDR) number.

 

·        Click the ‘Find’ button

 

 

Completing Record

·       Navigate through the system using the ‘Tab’ key on your keyboard (located on the left side of your keyboard)

 

·       Complete Items in yellow on Medical Screens 1-3

 

·       If a field is mandatory, it will remain yellow until you enter information in the field. (ig. Manner of Death)

 

 

·        Click on the ‘SAVE’ icon after completing each Medical Tab.

 

Medical Certification

·        Click on the Medical Certification icon to certify the record

·        Review the information

·        Preview Abstract of Record *(optional)  

·        Click ‘Certify’

·       Enter Personal Identification Number

 

 

Drop-to-Paper *(optional)

·       If Funeral Home requests because they are not electronic

 

·          Click on Icon to Drop-to-Paper

Using Work Queue to Retrieve Record *(optional)

·       Select Filter ‘All Unresolved’ under the ‘Select Filter Option’ Drop Down
